load the save and beat last boss without dying, if you die quit to main screen and reload the save again.
These are two files, one save file and one profile file which creates automatically as you start the game so I don't know if both of them are needed so it is up to you to resign one or both and try it out.
These are my saves and not stolen from somewhere

Kick *** dude! These were the 2 that I blew during my progress. Thank you so much! I haven't beat the game yet, but I will be using this as soon as I do.

Just beat the game. Loaded up your save and it worked like a charm! I thought it didn't work because it rolls through the credits and puts you in Easy+ mode. Only moments after that it popped 2 for 100GS. Thanks again man!

I have attached a save I made for the Strike Team and Upgrades achievements. This save is 1 Evade/Destroy away from both "Threat Elevated" and "In Plain Site" as well as 1 upgrade away from "Unnatural Selection". Just unlock the only upgrade on the first page. It is saved as a slot C save, so I don't know if a slot B save needs to made before trying this one or not.

OK here is my most recent save set. i have restarted the game to include some of the earlier cheevos i left out as well as save spots for the story based cheevos. follow the enclosed instructions since there are some cheevos that must be done legit (speed bumps, cleanup, the butcher...). i have made a save along with instruction on where i got these cheevos. this is my first save set so if there are any problems please pm me and let me know. also this set is 100% mine. i take all credit. Make sure to resign saves.

its a skill you have to unlock. once you have the skill in your disguise upgrades buy it. then walk behind someone hold RB and press B.

there should be instructions. for the infiltration you just have to infiltrate one more base. for the wrecking yard you need to destroy (or run over with a tank like i did) 6 more cars.
